Wooden Frame Wells Gardner 4500 Modified TV Chasis?

I've got this early Wells Gardner (edit: 4500 thanks to Ken L.) That appears to be a very early version, actually a converted television chasis, etc. I've taken some pics in the hopes of identifying it properly before testing for functionality. Everything appears to be in good shape with no noticeable damage or bad components.

I'm not sure exactly where to hook up the video signal, or what type of Sync it accepts, but I can hopefully figure that out with some help?

Also, I'm wondering if this should be run via an isolation transformer?

Have a look at the pics and if anyone has any information that could help, it would be greatly appreciated.

Thanks Ken! I've never had or worked on one of these before. I've found the schematic thanks to your sharp eye. Is this thing worth messing with?

We had one of those monitors years ago. It was so frustrating for me to work on that we scrapped the monitor and replaced it with something else.
 
Well, powered it up and its got a bad/shorted 2sc1454. I may have one around, but if not, I might just part it out or whatever. If I get it up and running I'll report back. Thanks again.
 
I have one of those tubes in storage(been meaning to recycle it). Not sure if I still have the chassis. I think I have seen one of those in a galaxian.
 
Anyone know what to check for no horizontal deflection on the WG4500? I was trying to hook up the input signal (which I did wrong since I hadn't seen this post yet) and the hz deflection shorted out I guess. Thought it would be cool to have this original type monitor in my Galaxian since it's so old school neat!
