It will have a sticker on the top edge of the frame that states the model #, but you won't see it without removing the entire monitor from the cab, and you have no need to do this.
Arcade Parts & Repair should have the cap kit. These have some uncommon values like 180 & 680uF, but I just buy them bulk from Mouser or Digikey.
These originally came with top-quality caps, but anything this old needs a rebuild.
There is never a need to swap a flyback on these unless it's actually bad, but I don't think I've ever seen a bad one. Maybe one.
