On what ever sideart may be under there, I can make out a circle shape in the upper right corner. It almost looks like a Centepede but the circle I described doesnt quite fit that sideart. Rumor has it that it was once a Tetris at some point...but what did this roll out of a factory as?

It was Kangaroo.

No, I can't tell, but Kangaroo got converted a LOT back in the day. I've seen several Kangaroo's converted to to Tetris, or to Arkanoid. It was a popular candidate because it the game was so bad.

I'm going with Kangaroo, the circle is in the same place as the Kangaroo art. Too bad, the owner wants to keep it Mame'd.

It's better off. Hate to say it, but of all the games to restore, Kangaroo isn't one of them. There are plenty of nice ones out there. And besides, he probably MAME'd it after it has already been converted a couple of times.

You'll always find Kangaroo either converted into something else (possibly more than once), or completely mint with low plays (since nobody ever played it).

Kangaroo is AWESOME, and much better than Centipede or Dig Dug. Quit the hatin' cuz you can't do fatalities...
