Based on the shape of the cabinet, can anyone guess as to what game this originally was? If not the game, how about maker? (i.e., Atari, Midway, etc.)

Betcha it's a custom cab.

It's kinda shaped like a dedcated Space Ace, but it's all wrong...that coin door sits mighty close to the cp and it just doesn't look right for commercial use imo.
 
Looks like a Lethal Enforcers that has had the sides cut to level the control panel.

I don't think its LE. Angle of the CP is wrong. Cut of the front bottom of the cab is wrong. Conversion people are lazy, they are not going to change the control pnael angle unless it has to be changed.

Seems pretty damn close to LE. I agree, it's not exact which leads me to believe it's a built-from-scratch cab off the plans from Jakobud that someone tweeked. Could be wrong though.
 
Doesn't look to bad. I will have to remember what you can do with one of those things if I see a dead one at an auction.

It is definitely not built from scratch, it is pretty old, it's been totally refurbished.

It is now a 3,025 in 1 with a 25" CRT. I changed the joy sticks to the classic red ball top type and I also added some retro side art, see enclosed pic of side art...
