I recorded this with my cell phone, so it's not the best quality, but it does show off what is going on.

I have run the tests and everything checks out OK. I've ensured that there aren't any loose chips on any of the PCBs, and there aren't any loose connectors either. Not sure what could be going on, maybe I need someone to take a look at the board more closely.
After viewing the vid, any ideas?

Thanks splattergatz. I tested and I was getting 4.25V, a little low. I adjusted the pot and got it back to 5V and we're all good.
