I'm gonna say I think that is a skinny neck tube. These are less common that the wide neck WG4600 type tubes.
No, I cannot say which TV you are looking for. What I do is open up each and every TV I find and cut the neck board out. I keep one of each of the common tube type neck boards handy. When I need a tube, I open up several TV's untill I find one that matches the neckboard I am hauling around. Of course, this assume a sizable supply of junk TVs. Find a recycler near you.
That's too much trouble. Most newer TV's have 10 pins, so you will have to find an older TV. Just make sure it's 19" and not 20". Take your neckboard and try to fit it on the neck. If it fits and the pin count matches, then it'll probably work...
