Ok, at first I just assumed I was bad at the game, but then I started feeling like there was no way the road signs could have been this confusing...take a look: https://photos.app.goo.gl/graGcwYwa9ySzzDi7
I've tried to cross reference about as many other YouTube videos as I can of mini ride on units and can't find someone with a similar issue. Now someone had patched my roms with the free play/enhanced setup. So they're definitely not original...but does anyone else running this rom set have these issues?
For those not following, it appears that my first 2 sets of road signs are wrong. They are on the left side and telling me to turn right despite the multiple left turns in the road. The third set appears fine, but then the fourth is on the right side and telling me to turn left despite the road going right. Fifth is fine, then the sixth is wrong...just weird. I did briefly try the senior course and it seemed fine. So it's not clear why it's so weird on this one.
EDIT: The original rom set that pairs with the early sub cpu roms has not been decrypted as far as I can tell...so if you're running the Super Hang On Enhanced roms while using an old sub cpu rom, you're going to have incorrect road signs (and presumably vice versa).
The easiest solution is to use the Sega Hang On Limited Edition roms which are decrypted and use the older sub cpu roms. Assuming you want the gameplay to be easier. Otherwise, update the sub cpu rom set to the newer one and it'll also correct the problem while retaining a more challenging layout.
