Every now and then, our first Rush gives me this message when it boots... but when I go into controls test, I can't find anything wrong with it. Is there some kind of intermittent failure I should be looking for, or should I not worry about it?

just replace the switch and actuator to be safe. Sometimes the actuators break and make intermittent contact.

check to make sure it's not the wiring too. I'd go into the switch test, maybe throw your switch wire on the NC terminal, and just wiggle the wires to see if there's a flash at all. and if you wanna check for broken grounds, if the grounds are in like a daisy chain, go to the first one that's not a splice-in to a switch you got on NC and wiggle there. then deduce along the line if any of the spliced ones may be broken.

I re-did a 6 speed shifter on a Ridge Racer that had the same problem, I just soldered the wires direct onto all new switches, and ran my own grounds and just connected the ground wire off the harness to the first switch.

and if this makes no sense, I'm sorry, I just woke up lol

Actually I think that Rush2049 might be COM-->N/C from factory. I know that Atari did that on CalSpeed but I also think they might have done that on Rush2049.

I'd replace the switch and the actuator.

that crossed my mind actually. I suppose switching it to NO would accomplish the same thing. I would go ahead and replace all the switches though, especially with all the mileage the game probably has on it from its former commercial use, or just use in general.

they don't cost that much to replace.
