Anyone know what causes this? Game seems to be running fine, it's just that the graphics look like this on the screen.

I've re-capped monitors, do you think I should try that first?

It could be an adjustment issue. Research your manual for tips.

Definitely check your sync. It looks like you might just need to adjust H Sync/Hold, but it could also be something like a bad solder joint, dirty connector, etc on the sync pin(s).
