Phoenix Monitor

This must be Phoenix issue week. The monitor has always had some issues with minor screen curl on the upper right hand corner of the screen, but now it is taking some time for the image to appear on the right side of the screen (It slowly draws in pixel by pixel). Also the color/brightness of the monitor is now taking some time to warm up. It starts out looking really dark and then slowly gets brighter (it was so dark yesterday I thought the monitor had crapped out at first). I adjusted the brightness, but have found it has returned to needing to "warm up" today. Can anyone diagnose these issues for me?
 
What is the make and model of the monitor? Has it been recapped lately?
 
I have never done any work on this monitor. Haven't had this machine for very long and the more sever issues just started. It appears to be the original monitor installed in this cabinet. Not sure on the model of this monitor so I have included some pics of the labels I could find.

Well, unfortunately, you have one of the oddball brands - a Toei CM-A20. I'd start with a cap kit and check for cold solder. If the flyback is the cause, then you're screwed, as they don't make them anymore...
 
A tube needed to warm up? I wonder if a recap job might clear up it's issues.
Caps drying out might cause a monitor to act that way.
Maybe the tube can use a trip through a crt rejuvenator.
