Thought some of the folks here might like to see a scan of the card that was in my Omega Race upright when I brought it home a few weeks ago. I'm assuming an op would complete this upon receipt of a game at a site, to ensure that shipping standards were acceptable for the games.

Would love to hear some op's takes on these, if you filled them out, if they got filled out with bogus info, if Midway would check up on sites if they didn't get the cards back, etc..

That's pretty cool! We use the same type of customer feedback advertising with our company's shipments. We average about a 5% return, but we do use the feedback.
 
Interesting how many times midway (and others?) mess up the names of their own games!

it said on it if I filled it out.. I would get promotional materials and some other stuff...

I thought "cool"! ;)


I sent it in... NEVER heard anything back... you think even a "not a chance" letter... but nope..

Did it go to the same address in IL? Do any KLOV members know if there's even a Midway office there anymore?

If it didn't come back to you, it must have gone SOMEwhere, and it says on the card that postage is to be paid by Midway, so do they just eat that then?
 
I have all the original paperwork in my Pac Man. There are references in the cabinet to "Packman". I like the paperwork also. Makes me feel like I was there when it was delivered new.
 
I have a few of those cards. I think from tron, ms pac, possibly Root beer tapper. Maybe spy hunter too. I usually just tuck them away in the manual.
 
I have quite a few of these also, some in absolute mint condition. I have no idea what I'll do with them...
 
i have all that stuff for my wizard of wor. never thought to send it in, though. 5 years or so ago, though, i found our old atari 5200 with all the paperwork, and sent in that warranty card. got it returned to sender.
 
The Midway office hasn't been in Franklin Park in a long time. According to Wikipedia:

The Bally/Midway division of Bally was purchased and reincorporated in 1988 by the arcade and pinball game company Williams Electronics Games through its holding company WMS Industries Inc. Midway moved its headquarters from Franklin Park, Illinois to Williams's then-headquarters in Chicago, and WMS reincorporated Midway as a Delaware corporation.

I'm pretty sure the "forward mail" request has expired by now :D

Business Reply Mail that can't be delivered is, if I remember correctly, eaten by the post office (i.e. they throw it in the shredder).

They don't return it because they've already lost money by having to handle it, and they don't want to lose anymore on the deal...
 
We never filled them out. Often left them in the games or threw them away.
