Are you sure about that? I always thought that it stood for Multi Dual System and Multi Game System.

After quickly checking all of the manuals, the two Dual machine configurations have serial plates as follows:

MDS-UP-US (Dual upright)
MDS-TBL-US (Dual cocktail, aka Red Tent)

The Single monitor UniSystems both are MGS (regardless of gun accessories):

MGS-UP-US (dedicated)
MGS-UP-US PAK (conversion kit)

All of the revisions of the PCB's are MDS model numbers.

So the PAK in the serial # means it was a conversion?
 
No it was a DK cab converted.

Sorry, not yours (serial no. 13754 PAK) -- I was referring to the image of the tag @apswann posted (serial no. 10523). AFAIK the "PAK" at the end of the serial no. indicates it's from a conversion kit (or conversion "pak" as Nintendo called them), rather than a dedicated cabinet.
