Looks to possibly be a Sharp Image chassie which looks to be a kortek hybrid of some kind.

Whats my monitor (Sharp Image 412??)
All I can find on it is Sharp Image XI-412 on the pcb it is 19 inch with remote adjustment board with degausser switch on it

I think I found it!
Found this post and based off being the same model on the remote board to the chassie and the 14-pin match.
