My New 1943 "Frankenstein" Cab. Need help identifying Monitor, Cab, and Video Issue

I know it's a lot to ask, but I figured I'd keep my initial questions to 1 thread.

I found this project cab on Craigslist for $130 today. I loved playing 1943 as a kid, and figured what the hell. I think the whole cabinet is a Frankenstein concoction, but it all seems to be in good shape. I have 3 questions about the cabinet, that I'd love some help with. It'll help me decide what direction to take with the cab. I'm pretty sure I am going to keep the configuration as is, but I'm definitely going to repaint and get everything in top shape.

First off, what cab was this? It's been painted over, but I took a little paint stripper and found there to be blue and yellow on a small spot. Is it a Pac-Man? The exterior sides, top, and front of the cab seem to be plywood; while some interior sections, the floor, and rear panel are particle board.

Second: I need help identifying the monitor. It's not original to the cab, because the manufacture date is 1991. The tube itself is manufactured by Goldstar, but there is a stamp on some of the metal frame that says "TeleBilt SAM-20C." The model # tells me it's a 20", but googling the model # gave me no hits. I've included a picture of the neckboard and Chassis, in hopes someone knows what it is.



Third: There is an issue with the video. The game plays and sounds great, but there is a series of lines that wave through the video. The whole image has a gentle wave motion to it, almost like a flag blowing in the wind. The lines in the image follow the contour of the wave motion. Is this more likely a board or monitor thing?



Any help would be appreciated. There are several parts inside the cab that have wires cut, and don't seem to be in use. I'm probably wrong, but is there 2 power supplies in this cab? A switcher and linear? Forgive me, but this is my second cab, and it's quite different than my Donkey Kong.

Yup, that's a Super Pacman cabinet.

The monitor is a Tekbilt/Imperial/Wico. Made by Kortek and sold under those names.
 
Thanks for the help so far guys, it's making more sense now. When looking for parts which name should I search for then. Are these monitors decent? It'd be nice if I had interior pics of a super pac and 1943 cab, so see which parts belong to what cab.

Any idea on the video issue?

Edit: Big Thanks Ken, I've found a picture of it on Bobs Monitor ID page. It's a Kortek K1-2-V0 board.
