Looks like Rev 4.0. The latest and greatest before switching over to the T-Unit. 4.0 is the only Revision that was on both the Y-Unit AND the T-Unit.
 
Strange i don't remember seeing one with a header at the bottom near the ribbon cable connector...

I just checked all mine and none of them have the headers. They all have the solder filled pads just like J6 and J7. Strange indeed.

Special? Perhaps, but other that having a connector at J9A/J9B, it looks ordinary to me. It's most likely a prototype of the newer version of Y-Unit board. The really early Y-Unit MK boards had "rocker-switch" type of dip switches and a yellow jumper wire running from the bottom of the board to the top.

Later on, they changed the design of the Y-Unit board, getting rid of the yellow jumper wire and changing the dips to the standard "on/off" type. I think this may be one of the early versions right at the change. That's the only reason I can come up with as to why there is a connection there and not on any Y-Unit I've ever seen before.

Anyone know if you can plug and play this board set with an original MK 2 cab? How does revision 4.0 differ from the final 5.0 revision?

Sorry OP back on track, I'm thinking that boards one of the old Y units or early versions. Those tend to have the extra header at the bottom like that.
