I think it's a Hantarex MTC9000, but it looks like there are a couple variations.

Bingo, Kortek it is. Now… Which model? It has some image curl and I need to figure out what the right cap kit is.
 
Still trying to figure out model this is. Here's a better photo of the chassis, right side of the image is to the rear of the monitor.

I emailed Bob Roberts, he doesn't know what it is.

Here's a tip man...

Grab a pen and paper, write down each cap value on your chassis. Download a few kortek cap lists (google is your friend). Then try and match up your list to a known one. Bet you'll figure it out!

Not being an ass.. This is how I do it when I'm having a tough time with chassis ID.

Yeah, I know. I was just hoping I wouldn't have to pull it apart until I was ready to fix it up.

Always need to check the connections
on any electronic board first!
then check other suspects like any obvious
caps that look bad!
Or large watt resistors not making good contacts.
after you clean it..so you can see the parts.
and components using large heatsinks confirm they are solder in good.
A good way to see which chassis it is, is check the flyback part#
then check the replacement odds are it will show you which chasiss
use it.

Well, I still have no idea what this is. Every Kortek cap kit I've found is laid out in Cxyyy format, this is Cy / Cyy.
