I have done some searches and have found no info on this Tube. The other numbers on this are Crt 345-14011 and yoke 345-14021. It is a small monitor 13" in perfect condition with no burn in, so I would like to be able to use it. Sorry no picture as I don't have a working digital camera.

Any suggestions as to where I might find some information so that I can buy a chassis?

You sure you don't have a typo in that tube number? I can't find anything in the B&K guide with that number.

I do see a 370LMB22 which uses a socket adapter CR41 (very oddball).

In the B&K guide the entire 370xxxxx series of picture tubes are all the map for adapters. Most are oddball.

EDIT:

After some googling around, I did find a Mitsubishi tube # 370MLB22. Many were used in medical monitors and it's a 14" CGA (standard) resolution.
According to the schematics in this service manual:

http://craneelectronics.com/Service/Monitor/HEWLE-A1.PDF

It uses a CR23 adapter! That's good.

Now measure the resistance of the vertical and horizontal sections of the yoke connector and we can find you a universal replacement chassis for it.

Wei-ya model 814H should work for your purpose. It will require an isolation transformer.

You can buy the above from Alva Amusement.

Granted the replacement chassis will have "phony" connectors on it and regular power cord that makes people think you can plug this chassis straight into a wall outlet. This must be run from an isolation transfor. Do not be tempted to just plug it in or you'll ruin your new investment immediately.

You'll be wise to "Americanize" the chassis before installing it. Here's more info:
