Ok - So I have checked voltages and adjusted (finally got the screen to read everything in English). ROM's and POKEY's were getting about 4.94 V but the test point at the edge connector was dead on 5.0. Bumped the 5.0 to 5.1 and all chips are now registering about 5.04.

So, I decided to take a video and post to Youtube. My monitor situation is unresolved, but I am not sure that it is the monitor.

I know that one POKEY is bad, as it shows up in Self Test and I am missing some sounds. Would not think that would be the video problem though...

Sorry everything is sideways! This is my first YouTube post.

Thanks for the help -

Damn dude! Get a good POKEY! Or at least turn down the volume while you're shooting a video. :)

I think you have a monitor issue but it would be a good idea to test your PCB in a different machine or throw a different monitor into your Millipede to be sure.
