icade error lights?

I'm building an icade 60 in 1 and it's not going well. A few weeks ago I plugged it into a cabinet with a good power supply and it wouldn't start up. I couldn't get into the test screen either.

LEDs 3 and 5 stay on with no change.

I took it out of the cabinet and plugged it into an NBA Jam and it was working, which was wierd because it wouldn't work in any other cabinet I tried.

I had a couple other jobs to do so I put it on the backburner until the other day. I plugged into the cabinet and of course nothing worked. I took it over to the NBA Jam and it's not working with that power supply now either.

Once, while over at the NBA Jam cabinet yesterday, LEDs 1, 2, and 3 started flashing one after another like there was some error. Of course the manual has no mention of any error codes.

Has anyone had this happen with the error lights? I'm thinking that the board has ended up failing, even though when we first got it in it worked. My boss thinks I should remove and reinsert the socketed square chip on the board, but I've never messed with a chip like that and I'm worried about damaging when I try to take it out (not really sure how to take it out anyway).0

As usual, I'm at a loss and appreciate any help.

Those boards are picky with the 5v. If it's a little low it will give you problems. Use a MM to check the 5v and see where it's at and adjust it if you can to like 5.02v.
 
I've adjusted the +5 as far as 5.20V (manual is saying not to go over 5.50) and no luck.
