I'll second the comment about likely bad RAMs, and add that the Pattern board can also generate similar faults.

You shouldn't die when you run into the scores left behind by shooting something, the collision detection can also be a symptom of board failures- usually RAM again.

I've got versions that do and don't depending on the code version. It seems to follow the same pattern as the placement of the heat sinks on the 40 pin customs.

There were also cabinet revisions that included a punched louver version of the lower bezel retainer, and the earlier non-punched version. Those seem to be found in the same cabinets as the chip heat-sinks.
