The Dave Ramsey "Total Money Makeover" has helped us immeasurably.
We are down to only the balance on the primary mortgage and the remainder of the med loan, which is now smaller than the mortgage - no other debt of any kind, no cards, no HELOC, no car loans, nothing.
200K grows terrifyingly large, horrifyingly fast while you grind through residency and then fellowship, I can tell you from experience. We're on the downslope now, that's for sure.
