sounds like your power supply isnt putting out the right voltages

check all your ground wires. make sure they are clean and tight
could be the widget board is bad, making the controls not work or again, a bad/no ground

rebuilt power supply and putting out good voltage. +4.9v -5.1v +11.89v. should be close enough to work right.

Should be a bit higher on the +5 epically if you are not measuring at the chips. I usually put my +5 to 5.15 on the furthest chip.

That +4.9 will drop further from the connector.

Maybe the small Widget board the control panel is hooked up to is messed up. The big 6821 chips can go bad. Usually when they do, a portion of the game controls will not work.
Good luck on getting it going.

the widget got traded out from eldorado, but who knows it could have been on its last leg.

Go into diags if possible and run the switch test. Sometimes a bad widget board will show up as continuous button events. But that said, my bet is on bad grounds.

The coin door is run off the PIA (6821) chip on the ROM board not the I/O board.

Try the Switch test if you can get into it. It may just need the connector replaced with a crimped Molex connector instead of the old IDC connectors. Try wiggling the coin door connector while you are in the switch test. Or the header on the ROM board may need to be reflowed.

turned out the widget board was bad, hooked up a good one and the game works fine. send this one back to eldorado for my money back.
