Any ideas. Have been comparing it to many midway cabinet styles and can't find any close. Has dark blue paint overspray inside the cabinet. Also attached are a pic of the power supply and attached label inside.

The cutout doesn't match Super Pac, and it doesn't look like it was modified (which also wouldn't be easy to do well, plus what would be the point in doing so?)

That was my thought. Looks very similar but the cuts are very good and without stripping it I don't know if it is original. The blue does look to match the super PAC though.

Midway type "SUPM-UR" ID label.
Very similar outline, marquee size, and monitor orientation etc, IMHO.

I also vote that it's a Super Pac.
 
I am sure someone out there has a super PAC and can take a pic of the tag on the top side of the inside to verify that it's a super PAC. Or maybe I have a super rare prototype cabinet. ;-)
 
I can accept the tag as confirming it (assuming those who are claiming so, also have a Super Pac with the same tag), but that modification doesn't look homebrew.

Of course that could be wrong, and someone with a clue actually did it well, but that still seems unlikely to me anyway. That t-molding also looks old (i.e., not the typical new repro stuff), but it is hard to tell from the pic.

I wonder if you pulled the t-molding off (or back), if you'd see cuts in the barb for where the old bends used to be.
 
So the story is my neighbor has had this for like 10 years. Stopped working a few months ago and didn't want it anymore and asked if I wanted it. So score 1 for me. Now I have to decide if I just clean up the conversion or bring it back to life since it has already been modded and I am not sure I want to invest that kind of effort in graphing plywood sides on it to get it right.
