Need to fill a 3x4" gouge about half the depth of the particle board in the side of a cabinet up near the top. Is it okay to fill with patching compound (fiberglass bondo, wood patch, spackle, etc.) without treating the MDF surface first? Is the moisture in the patch enough to make the remaining MDF swell (and possibly break on the other side)? Or am I overthinking and bondo/sanding/painting will be fine?

Yep, Bondo, sanding, painting will be fine as long as you have enough solid substrate to adhere it to. I usually treat the offending surface with wood hardener then apply bondo about 30 minutes later.
