Anybody know what game this Mr. Do was?

Go get it, fix the monitor board and you'll have a good game. I owned that game for 10 years and never had a problem with it. It was left on for 3 months 24/7 at RQ, which is why the board fried. I'd buy it back if I wouldn't have bought another one already.
 
That machine kicks ass. If it was the first week in August, I would have already bought it.

There is a small possibility it could be a converted Ladybug with a full Mr.Do art package... Both Ladybug and Mr.Do dedicated cabs had those stripes around the coin door.

No, it was converted from this. But "Tree" isn't a very good game anyway.

And yet, that is also a conversion from this:

But what's amazing is that if you look carefully, you can find the telltales signs that tell you even THAT was a conversion:
