25" Polo blowing HOTs

Subject says it all.

Got a 25" polo... blew the hot out (completely open on all 3 pins).

Bought a new HOT, put it in... ran perfect for about 15 mins, then the HOT went out again.

Ironically enough, the HOT was... well, hot. VERY hot. Touched it after the monitor shut down and it burned my fingers pretty good.

The flyback was replaced when it was recapped.

What should I be looking for?

I'm just going to order in a dozen or so HOTs and try to figure this thing out... but I'm really needing some suggestions.
 
Silicone grease applied to the new horizontal output transistor?

Mica insulator between the transistor and metal frame?

Damper diode replaced?

#2 right there could be the issue. The transistor was already replaced when the "first" one blew. There was nothing between the transistor and the frame, just the grease.

As for the diode, no... it was not replaced. Can I get a # for that so I can order a few of those in as well.

This chassis has been a nightmare... really just want to get it fixed and call it done.

Since there in no insulation between the transistor and the frame, do you know of anything that I can put back there to substitute?
