The motherboard is the P4M800PRO-M478. I do not have a picture of it and it is in storage at the moment. I do however have a picture of the screen where it stops loading that I took a while back.

I was able to get a copy of the disk image from CosmoDog a while back but was never able to successfully transfer it to a USB drive properly so now the game tries to load but gets hung up at some point which is shitty because it is a cool looking piece.
