It looks to me like blanking is not occurring when it should be during retrace. If a monitor hasn't been gone over yet, I would do standard preventative maintenance before thinking too much harder about it (aside from the caps, which you have done already).

Is your video grounded? A very long indirect ground will not work in your favor, so make sure that is coming in alongside the R, G, B, and Csync connections.
