BAD HARDWARE is probably the MCU 68705. It has a prom in it so it could be faulty.

Another option worth considering might be to use a larger AM6012 DAC (in the open slot) and populate the supporting resistors just like Asteroids Deluxe. Only worth trying if you really can't get the ad561j DAC though.
