    I've got the same exact issue.. EDIT: I spoke to soon.. checked the +5v and it was coming in at around 4.6v. I upp'd it to 5.3v and voilà! 8)

    OK.. I got the machine. Ends up a new PSU was recently put into the machine. It measured at just under 5.0v, so I bumped it up to 5.07v, but no change. I even tried bring it down to around 4.85v, same issue. Double checked all the connections, everything looks good... ugh.
