It's a Big Buck Hunter II: Sportsman's Paradise with a Zenith monitor. A label says the tube type is A68AEG25X07. Not sure if there are other markings but that and the serial number were all I could see without removing it.

Yes! Sorry, it took me a while to get back there. Adjusting the rest of the edge connector pins resolved all the issues! Well, except for the dead battery on the board :)
